[Futuregen] Cohort paper

Dear Stefan and Johan,
Attached the document with a few additions form my side. I tried not to
replicate any of the previous comments and made mostly punctual
suggestions.
On the issue of simplifying the story line I can think of two
potentially helpful approaches:
- focus on either ADL or IADL and report the other only when there is a
marked difference
- add to the intro a short statement of the research questions or
specific study objectives that can structure your presentation of results.
I would suggest focusing on comparison of trends within regions rather
than comparison between regions, mostly because I am concerned about the
data quality. I think this is partly Ricardo's suggestion as well but
based on a different argumentation.
I hope this can help. You certainly have a great paper already.
