Thank you very much for your comments and edits! I accepted or/and addressed most of them in the new version attached. This version also contains the Discussion section and is therefore the complete document for now for the Transforming Care Conference.

If you have time, please read the the new Discussion section. You can of course read the rest as well to check the changes that were made, if you have additional time. Please bear in mind that the abstract has not been changed (this will be done at the end), nor the references fully updated.

Stefan&Johan, we checked the SHARE documentation and got quite different response rates from the ones you reported  in your study on disability trajectories. We include these figures in the Discussion (marked with a
comment) and in the Annex. Any ideas on why the discrepancy?

You will find a version with track changes, in case you want to see the changes implemented (including your edits), and a "clean" version with all track changes accepted and only the comments in place. Please use the latter for any additional changes or comments and if possible mark the new comments with "NEW".

Please check also your affiliation details. For now I included as co-authors all those who provided comments and edits.

It would be great if you could send me this by end of the day of Friday, please. I will then take care of the final edits before uploading the document.
