Hi all,

Sorry for the delay in sending the manuscript Susan and I, in 
conceptual collaboration with Ricardo and Stefania, prepared.I am not 
going over technical details of the analysis but I have done enough 
iterations of analysis and can tell that the generated trees are 
reliable. Most of the issues that you see in  comments have already been 
resolved but I left them there just  in case they are your concerns as 
well.

Please have a look specifically:

1) to see the flow of the introduction makes sense
2) whether the results section is appropriate (we can explain any part 
of the tree that we like but I tried to focus on location that I thought 
are more important)
3) discussion section is in a point format for now, add additional 
points you think is needed

Attached: the last version of the manuscript (v4); two regression trees, 
one for formal, one for informal care (PNG files)
